Nina W. was wonderful in providing me with all the information and resources available through this program. The food gift cards helped offset the grocery bills during our stay in SF, and the parking vouchers were a tremendous help in cutting some of the costs of our appts at the Van Ness Campus! Although we didn’t take advantage of the Lyft services, it was nice to know that was also an option. The services and support relieved a lot of the stress so I could focus on my recovery. Nina really went the extra mile and stayed in contact with us the whole time we were in the CPMC apartments to make sure we had whatever resources we needed through the program. I would definitely recommend Health Navigator Foundation! Thanks!

This program was incredible! They provided resources, weekly check ins and kind words to help me get through the very challenging early parts of recovery from my surgery. I am and will forever be grateful for the additional layer of support that this program provided for me and my family. Having extra resources provided to help with groceries and everyday expenses as we adjusted to my new post surgery life is something that was immensely helpful for me and all of my family. I can’t say enough good things about the program.All of the doctors and nurses were wonderful to me and helped me get through trial in my life. I thank Doctor Herr and his team at CPMC for saving my life.My transplant nurse Noelle was so helpful and compassionate to my needs, I commend her and she deserves a huge raise and accommodation for her care for me.She was always there and helped me through my emotional struggles with my depression.Nina,my nurse navigator also was fantastic with her help and support during my needs. She was also always there with support, encouragement, understanding and her endless compassion. I thank CPMC and Sutter for saving my life and I will be forever grateful to the whole staff and I pray for all of them. Thank you and God bless you All.

I am so appreciative of this program! Having a Iiver Transplant was a great blessing and comes with a lot commitment and work that I am happy to do. It is hard and rewarding and my recovery is going amazing and I’m healing faster than expected. Help with gas, food and supplies gave me great relief. Ms.Nina Wi. and the Health Navagation Foundation. They have truly been a blessing. I was turned down going into a acute rehab facility by my insurance Co. This caused me to have to travel 50 miles, one way, from Antioch to San Francisco. So this is equivalent to 100 miles per day, round trip, twice a week. The Foundation has provided me with gas and food gift cards. What a well needed blessing! I had a Kidney and Heart Transplant November 19-20, 2022. MSs. Nina also connected me with a person who can walk me through my post-transplant period which is priceless! I highly recommend the Health Navagation Foundation to anyone who would like to sow a seed into good ground! Very professional!

It was overwhelming from the start to know that there were so many things my daughter and I would have to handle and deal with for my monthlong recovery post liver transplant. The HSN program helped us before we even started with the first lab and clinic visits. Ana and Nina both helped us greatly by reaching out and assuring us that help and assistance was on the way. We don’t live in SF so the coordination of lyft rides until transportation was secured was greatly appreciated. We appreciate the financial assistance with the Safeway gift certificates that helped greatly with my nutrition needs for appropriate food ( Diabetic needs) and Glucerna that all of the MD’s recommended. I had a very challenging time post transplant with having to be admitted twice for emergency when I needed two blood transfusions. The assistance we received for my daughter to help pay for parking for those all day emergency visits and hospital stays along with the numerous clinic visits was also highly appreciated. Both Ana and Nina were so welcoming and nurturing which is what we really needed during these trying times. I had a kidney transplant here theee years ago and there was no post op required. I’m so grateful that this program exists and hope that others will also be able to benefit from all of the services offered. Thank you.

My heart is blessed by watching and being in the program. The Navigator Program has been so helpful and finding places for us to stay so we could come up here, we couldn’t afford it otherwise. Health Navigator Foundation has been an invaluable resource for me during a time when I was ill and didn’t always know where to turn for information or resources. The support we received for essential items such as transportation, planning, and even assistance with food were extremely helpful. Nina, our coordinator, was always available to answer questions and assist with any issues we had.I am grateful that this service was available for us!

The program as it’s set up right now is best you could get! The most helpful was accessing the parking vouchers and gas cards as gas prices have risen tremendously during this time, and driving to the city can be very expensive.

HNF is spectacular! I cannot tell you how much this was a blessing for us, especially for my wife as the caregiver to just have someone to talk to through the process. The resources for food and transportation were also a life saver as we have to commute over the bridge, in traffic, and the costs for food and parking in San Francisco can be so stressful. I can’t tell you how much this saved us in this difficult time. HNF and Ana are a blessing, and thinking abut her support makes me want to cry because of how much we appreciated her checking in and offering her services through the program. We swear by this program and cannot express enough how helpful this was for our family. With zero hesitation, we would highly recommend the program for anyone going through the LVAD process.

Helped me at discharge which is the most stressful time. Get one through the first month with food, gas and Parking vouchers for many medical appointments. Helped put food on the table and also helped me with gas and parking fees.

Support with transportation and food was so helpful given that we had to stay 5 hours away from home during the recovery. And although Ana and I speak different languages, her communication was excellent and translation tools worked seamlessly. I know that without the support of Ana and HNF resources, this would have been a much more challenging and difficult situation so I would recommend it to anyone going through the transplant process.

It was great to have a single helpline to call for any issues related to transplant across departments. Nina followed up frequently to check status and helped us in contacting [another patient] and his family who went through transplant earlier. We are also trying to contact the donors family through the health navigator. As I mentioned earlier, having a single point of contact is a great thing. Special thanks to Nina, the coordinator from health navigators!

I can’t wait until I can get out and play tennis again but I need to build my strength and energy back up. It was wonderful to have someone help talk us through the process and what to expect along the way in terms we could understand. They were always there checking on both of us making sure that no only was I the patient doing well but so was my caretaker. The caretakers often get lost in the process and if they are doing well then your care is the best that it can be. The program worked with both of us and cared.

My transplants and the support of the Navigator team has been instrumental in giving me a new life-quite literally.Navigator has made the whole experience as stress free as it could have been for both me and my carer.I have felt confident of progress and a successful outcome as a result of the support I have been given throughout,and the explanations and feedback that has always been available meant that I was never in the dark’ as to what to expect or might happen next.

Receiving a new heart when the old one was failing is akin to being reborn. Life appears much grander today than it has been in years. No longer am I looking at the future in finite terms. Life has the promise of being infinite and the future unwritten. But, getting here wasn’t easy. Making life-altering decisions without knowing what to fully expect is scary to say the least. Healthcare Navigators provided the much needed information of what is to be expected and, most importantly, help with all the critical details to ensure that post-transplant needs are arranged and met. It was not only a relief to me as the patient, but also a tremendous support for my wife who was my caretaker. It allowed the patient and caretaker to focus on recovering and not the peripheral obligations.
